Your first visit

With plenty of time and openness for your personal concerns, we await your first visit to our practice. In order for us to gain a meaningful picture of your medical history, it is important that you fill in the patient history form beforehand. You can do this in advance: Simply download here, print, fill in conveniently at home at bring it with you to your appointment.

Any documents from your doctor, dentist or orthodontist, together with any x-rays, are also very important for our initial discussion. The same also applies to medication that you take regularly, together with possible allergies about which your allergy pass can provide information.

In this way we will have a meaningful foundation for an initial personal and comprehensive consultation.
